How to Tell us about a Change of Contact Details

It is essential that we are notified of changes to your name, address, telephone or mobile number.

If you move out of our catchment area

If you move out of our practice catchment area it is important that you register with a doctor’s practice local to you to ensure that you can easily access primary, community and secondary care services close to your address. It enables better continuity of care as care networks are based around geographical areas.

Once we are given notice that you have moved out of our catchment area, we will give you 30 days grace to give you time to find and register with a doctor in your new area. After 30 days we will start the process to remove you from our list.
